Prior to a patient’s discharge from our facility, our rehabilitative and social work professionals collaborate with the individual to make sure they’re ready for any challenges involved with returning home or to an alternate level of care.
In addition to outlining personalized exercise plans and guidelines for safely performing daily routines, our team will ensure each patient can manage stairs and other obstacles related to returning home. Our social workers may also arrange for additional care that may be needed at home, such as therapies, nursing care and homemaking services, as well as wheelchair assistance or other assistive devices. Additionally, our nutritionists may go over dietary issues that could impact a full recovery and our nurses will provide information on wound care and medication management.
All of this will ensure a smooth transition and enable patients to resume optimal functionality and independence.
